In the United States, women receive conflicting advice about when to wean their children completely from breastfeeding. The American Academy of Pediatrics recommends one year, while WHO and UNICEF recommend at least two years. Many physicians consider six months to be "extended" breastfeeding, and some health professionals question the motives of women who nurse for more than a year. In turn, women may hide the fact that they are still nursing an older child from disapproving health care professionals or family members. From anthropological research, we know that in many non-Western cultures children are routinely nursed for three to four years. Are they eccentric, or are we? Can we look to other animals to determine what the natural age at weaning would be in modern humans if it was not modified by cultural beliefs?

Like all mammals, humans have mammary glands that function to nurture their offspring. Within the class Mammalia, humans are members of the order Primates, and have the basic primate pattern of breastfeeding and weaning activity that has been molded by more than 65 million years of natural selection to ensure the best possible survival rate of primate offspring. This basic pattern is assumed to be primarily genetically based. In addition, a number of life-history variables are also associated with age at weaning in the non-human primates. What do these variables suggest about the "natural" age of weaning in humans?

Weaning according to tripling or quadrupling of birth weight

The idea that mammals wean their offspring when they have tripled their birth weight is widely reported in the breastfeeding literature (Lawrence 1989). This rule of thumb holds true for small-bodied mammals, but not for larger ones. Recent research has looked at age at weaning and at growth among large mammals, including primates. The research shows that weaning occurs some months after quadrupling of the birth weight, rather than tripling (Lee, Majluf and Gordon 1991). When do U.S. infants typically quadruple their birth weight? For males, the average age is around 27 months, and for females, around 30 months.

Weaning according to attainment of one-third adult weight

Other studies suggest that primates are like other mammals in weaning each offspring when they reach about one-third their adult weight (Charnov and Berrigan 1993). Humans come in different sizes, but 4 to 7 years of nursing would be the weaning age for humans using this method of comparison, with boys generally being nursed longer than girls, and large-bodied populations nursing longer than small-bodied groups.

Weaning according to adult body size

Harvey and Clutton-Brock (1985) published a study of life-history variables in primates, including a formula for calculating age at weaning based on adult female body weight. The equation predicts an age at weaning for humans at between 2.8 and 3.7 years, depending on average adult female body weight, with larger-bodied populations nursing the longest.

Weaning according to gestation length

It is often reported in the literature that, among mammals in general, weaning age is approximately the same as the length of gestation (Lawrence 1989). By this criterion, weaning in humans might be expected to take place after only nine months of breastfeeding. However, this one-to-one relationship is greatly affected by the adult size of the animal. For many small-bodied primates, the duration of breastfeeding is shorter than the length of gestation. Among large-bodied primate species, the duration of breastfeeding far exceeds the average length of gestation. For humankind's closest relatives, the chimpanzee and the gorilla, the duration of breastfeeding is more than six times the length of gestation. Humans are among the largest of the primates, and share more than 98 percent of their genetic material with chimpanzees and gorillas. Based on these comparisons, an estimated natural age at weaning for humans would be a minimum of six times gestational length, or 4.5 years.

Weaning according to dental eruption

According to the research of Smith (1991), many primates wean their offspring when they are erupting their first permanent molars. First permanent molar eruption occurs around 5.5 to 6.0 years in modern humans. It is interesting to note that achievement of adult immune competence in humans also occurs at approximately six years of age, suggesting that throughout our recent evolutionary past, the active immunities provided by breast milk were normally available to the child until about this age (Fredrickson).

Our evolutionary past has produced an organism that relies on breastfeeding to provide the context for physical, cognitive, and emotional development. The human primate data suggest that human children are designed to receive all of the benefits of breast milk and breastfeeding for an absolute minimum of two and a half years, and an apparent upper limit of around seven years. Natural selection has favored those infants with a strong, genetically coded blueprint that programs them to expect nursing to continue for a number of years after birth and results in the urge to suckle remaining strong for this entire period. Many societies today are able to meet a child's nutritional needs with modified adult foods after the age of three or four years. Western, industrialized societies can compensate for some (but not all) of the immunological benefits of breastfeeding with antibiotics, vaccines and improved sanitation. But the physical, cognitive, and emotional needs of the young child persist. Health care professionals, parents, and the general public should be made aware that somewhere between three and seven years may be a reasonable and appropriate age of weaning for humans, however uncommon it may be in the United States to nurse an infant through toddlerhood and beyond.

How do we cope with colic?

Although colic isn't necessarily a sign that your baby's in pain, take her to your GP if she's crying for long periods. Your doctor can check if anything else is causing your baby to cry. 

In most cases of persistent crying, the GP will find that there is no underlying cause. You may start to feel more relaxed about your baby's crying once your GP has confirmed that she's fit and healthy. 

So, when your baby is having a bout of crying, just take a deep breath and carry on with the basics. Feed her on demand, wind her and change her nappy when she needs it. Try the soothing techniques that usually work well for her, whether that be singing to her or taking her for a walk in the pushchair.

Bear in mind that your tried-and-trusted techniques may not work every time. Colic means that, even though your baby is healthy, she may just cry for no obvious reason. While this can be stressful, remind yourself that you haven't done anything wrong, and that this phase will pass. 

Dealing with colic can be tough, and some parents feel depressed, helpless and even angry when faced with their persistently crying baby.

If your partner is at home, you could take it in turns to give each other a break. But if you're alone with your baby, you could:
  • take your baby for a walk so both of you can get some fresh air
  • ask a neighbour or friend to babysit while you have a bath or lie down in a quiet room
  • phone a close relative or friend to talk about what you're going through
  • cuddle and rock your baby, but put on headphones and listen to happy music, so that the only sound you hear isn't your baby's crying
  • If you can't get out of the house, do some exercise or dance to a games console or DVD

If the tension mounts to the point where your stress turns to anger, put your baby down in her cot for five minutes and go into a different room to calm yourself. The last thing you want to do is to direct that anger towards your baby. If you need to, set a timer so you know how long it's been. 

When your baby is calmer:

  • Have a rest, or some "me time", while your baby is sleeping.
  • Listen to a relaxation podcast.
  • Enjoy your baby by making a note of the good times you have together.
  • Talk to your partner about how you're both coping and what you can do.
  • Chat with other mums who have babies with colic in the BabyCentre community.

If you're struggling, it's important to seek help before you become overwhelmed. If you become anxious, your baby may pick up on this, making her more difficult to soothe. Don't be afraid to talk to your health visitor or doctor about how you feel. They're there to support you.

If the baby's stool is green and contains mucus, it could be due to some viral infection. Teething is another cause for this condition. This is due to the increased drooling that causes ingestion of excess saliva, which irritates the intestine and results in expulsion of mucus along with stools.

Food allergy is another cause for mucus and blood in baby's stool. Even breast-fed babies may have food allergies, which could be caused by the food consumed by the mother.

One of the serious causes for this condition is intussusception, which is characterized by intestinal blockage, caused by another part of the intestine itself. In such cases, jelly like stools will be passed by the baby, who will experience intermittent pain and vomiting too. The stool will contain mucus as well as blood and this condition requires immediate treatment.

Infections of the bowel (both viral as well as bacterial) can be among the potential causes for mucus and blood in baby's stool. In some cases, mucus in stool may be due to intake of fruits like, bananas.

In short, mucus in baby's stool without other symptoms like, fever, pain and change in stool color, is not a cause of worry, if the condition is not persistent. However, it is always better to consult your pediatrician, who will be the best person to analyze the condition. If the mucus is red, contact the doctor immediately. It could be due to various reasons, some of which can be serious.

Treatment for the condition depends on the underlying cause. While in some cases, the condition may subside without any treatment, some others may require treatment as per the underlying condition. 
 
While intussusception is often treated with immediate surgery, the condition (mucus in stool) will subside on its own, if caused by teething or viral infection. So, if you detect this condition in your baby, don't panic. If the baby is having other symptoms, seek immediate medical attention. If there is blood in baby's stool, along with mucus, then also immediate medical attention is required. If the baby is otherwise healthy, then wait for a day or two and then contact the doctor, if the condition persists. The above said is only a brief overview about mucus in baby's stool. Headlines are screaming about bisphenol A (BPA), a noxious chemical used in many plastic containers including baby bottles, water bottles, sippy cups and other containers used for food and beverages. Much of the recent clamor occurred after publication of a recent study, Baby's Toxic Bottle: Bisphenol A Leaching from Popular Brands of Baby Bottles.

The study, commissioned by Environmental Health groups in the U.S. and Canada, has resulted in calls for bans on BPA, an environmental estrogen, since it is known to leach out of the plastics and has been implicated in development and reproductive health risks.

Dr. David Carpenter, a professor of environmental health sciences at the State University of New York at Albany School of Public Health, said that BPA taken into the body before birth or in early years of life can alter the ratio of sex hormones and affect development.

"It's absolutely obscene to use a substance that can make little boys less masculine and opens the chance that little girls will go on to develop breast cancer," he said at the teleconference.

The BPA studies call for a ban on the use of BPA following action by the State of California to ban the use of phthalates and other known cancer causing ingredients in products designed for children. Phthalates (pronounced thah-lates) are used to make plastic 'soft' and is found in toys, teething rings, some bottles and other play things that children often handle or put into their mouths. The California legislation is leading other states to begin regulation on known carcinogens, hormone disruptors, neurotoxins, but the road to success is rocky and very long.

Early 2008, another study found elevated levels of phthalates found in the urine of babies who'd been recently shampooed, powdered or lotioned with commonly used, commercial baby products. These reports, along with a myriad of other recent studies, are contributing to real concern and outrage on the part of parents across the nation.

When the very products that are designed to encourage good parenting become a threat to good health, it creates worry and distress. It also points to the need for parents to become vigilant consumers who learn to read labels, follow new studies as they are published and begin to demand safe, clean products for their children.

One of the first and most controllable items that a parent can focus on are the skin and body care products they expose their child to each day. And, no, I'm not talking just about the baby care products. I am including all the items adults use to bathe or shower, brush their teeth, plus deodorant, lotion, makeup, nail polish and products that clean / freshen the home. Baby Care - Changing A Diaper

Ideally, a baby's diaper should be changed both before and after nursing. But until the age of about six weeks, most babies protest loudly and tearfully against anything that delays nursing time. So, provided that the baby has not had a bowel movement, it is better not to insist on a pre-feeding diaper change in the interest of a peaceful nursing session. After the baby has nursed, he or she is generally more cooperative.

Some parents, especially mothers, like to change a newborn baby on their laps; others prefer to use a waterproof changing mat on a bed or a dresser top. The mat should be covered with a soft towel so that the baby is not placed on a cold plastic surface.

No matter how clean the baby is kept, the skin in the diaper area may still become sore and red. When the first signs of diaper rash appear, add extra absorbency by folding a cloth diaper in half and wrapping it round the baby's waist over the clean ordinary diaper. Secure the extra diaper with a diaper pin, like a skirt. During a bout with diaper rash, plastic pants are no longer suitable.

To prevent a cloth diaper from becoming stained, rinse the stools off the diaper as soon as possible. Hold the diaper under running water in the toilet bowl. It should then be put to soak in a sterilizing solution, in the receptacle provided by the diaper service, or in a household bucket.

The parent's hands should be washed thoroughly after a diaper change. It is not appropriate to leave the child unattended for this because some infants will begin to roll over as early as two months. Here again, careful planning is the key in baby care.

The Older Baby. A baby who sleeps through the night may, during that time, soak through an ordinarily folded diaper. Soaking can be prevented to a certain extent by folding the skirt-shaped diaper around the baby's waist. But if the baby moves around the crib during the night (as happens when a baby is about nine months old), the skirt diaper rides up out of position.

To provide a diaper that stays in position and also has plenty of absorbency, fold one diaper lengthwise until there are three folds; turn up one end of the diaper to create six thicknesses, and lay the folded diaper inside the kite-folded diaper. This puts eight layers of diaper where it is most needed. Because the diaper is bulky, this method should only be used during the night, not when the baby is awake and active.
